Realistic expectations by recipient group

Implanted early within the sensitive period, a young child can develop near-typical spoken language, but only with intensive, sustained habilitation — the highest rehab demand of any group.

No two recipient groups should be promised the same outcome. A young child implanted early may reach near-typical spoken language; a prelingually deaf adult more often gains sound awareness than open-set speech; single-sided-deafness and bilateral recipients gain most in localisation and hearing-in-noise. Matching the promise to the group is the heart of honest pre-operative counselling. Schematic; scores illustrative.

Roughly 42% of candidates set a pre-operative expectation above the typical realistic 12-month result (here ~64% of speech understanding restored). When the dashed blue needle overshoots the solid green outcome, the shaded gap is the disappointment the consent conversation must close. Counselled candidates with lower, well-calibrated expectations report higher satisfaction — managing expectation is itself part of the treatment. Illustrative.

An implant reliably restores access to speech in quiet, environmental awareness and lower listening effort — and for many, usable phone use. It does not restore normal hearing: music and hearing in background noise stay hard, and “perfect hearing” is never on offer. Setting this baseline before surgery is the heart of honest counselling.
